Accountable to: QMRP and Charge Nurse
Under the direction and supervision of the QMRP and Charge Nurse, the Direct Care Staff is responsible for assisting clients with their activities of daily living (ADL’s), assisting the client in reaching his or her maximum potential in accordance with the Individual Service Plan (ISP) and provides for the client’s comfort, appearance and safety.
Qualifications
- Ability to understand and carry out written or verbal directions in English.
- Ability to document and maintain medical records.
- Ability to work a rotating schedule.
- Previous experience as a Direct Care Staff is desirable.
- Ability to lift fifty pounds.
Duties and Responsibilities
- Assists developmentally disabled clients with active treatment programs in accordance with their ISP and records data collection.
- Provides basic care for clients including bathing, feeding, grooming, dressing, continence care, etc.
- Provides suitable environment for the clients by maintaining cleanliness, ventilation, lighting, etc.
- Maintains client’s wheelchairs and other adaptive equipment.
- Protects clients’ rights including dignity, respect and confidentiality.
- Reports changes of condition to the Charge Nurse.
- Performs selected nursing procedures which do not require professional evaluation.
- Maintains company equipment and supplies in accordance with established policies and procedures.
- Monitors clients continuously for their health and safety.
- Documents Daily Flow Sheets, behavior data collection, changes of condition and other records as required.
- Attends in-service training and participates in fire and evacuation drills.
- Maintains a professional attitude at all times. Assists co-workers when requested.
- Assumes other duties as assigned by the QMRP and Charge Nurse.
